To qualify, you must:
* Be 65 years of age or older
* Be enrolled in Medicare Part A and B
* Live in the service area
* Meet clinical eligibility requirements (for example, need help
with daily living activities like bathing, dressing, eating, toileting,
transferring or walking/mobility or have a chronic condition
requiring nursing services), and
* Meet financial eligibility requirements (Medicaid eligible up to the
Institutional Care Program Level.)
